Udaipur : Geetanjali Medical College & Hospital has been a boon for the successful operation of aortic aneurysm in recent times. The two recent operations operated are the success cases for the first time in Southern Rajasthan and that too in Udaipur’s Geetanjali Hospital.
Geetanjali Cardiac Centre’s Cardio Thoracic & Vascular Surgeon Dr. Sanjay Gandhi, Neuro & Vascular Interventional Radiologist Dr. Seetaram Barath and Cardiologist Dr. Ramesh Patel successfully treated a 58 years old female which was having a deadly aortic aneurysm. The operation team includes Cardio Thoracic & Vascular Surgeon Dr. Reenus Demel, Cardiologist Dr. C.P. Purohit & Dr. Harish Sanadhya, Cardiac Anesthetist Dr. Ankur Gandhi, Dr. Kalpesh Mistry, Dr. Manmohan Jindal and Dr. Dharamchand.
Dr. Sanjay Gandhi said that the patient Santosh Devi (Age 58 years) was suffering from high BP since 10 years. She came to hospital with complaint of headache, bleeding from nose and back ache. On investigations she was found to have a dissection of the descending thorax aorta with aneurysm measuring 65 mm which normally is 2 to 2.5 mm. This aneurysm could rupture anytime leading to torrential internal bleed and sudden death of the patient. On discussion with family relatives about the disease and complexity involved, the relatives decided for further treatment in Geetanjali Hospital only. There were two options available for the treatment of the patient. One was Hybrid Endo Vascular and other was open surgery. Since open surgery involves more risk to life it was decided to operate the patient from Endo Vascular procedure which lasted 3 hours. In this procedure a major artery from groin was exposed to surgical and through this a large aortic stent was put inside the dissecting aneurysm and the aneurysm was excluded from the main lumen recovering the patient very well.
In other case, a patient of 55 years of age was treated from ruptured aortic aneurysm. The team members were Cardio Thoracic & Vascular Surgeon Dr. Reenus Demel, Cardiac Anesthetist Dr. Ankur Gandhi, Dr. Kalpesh Mistry, Dr. Manmohan Jindal and Dr. Dharamchand, Nephrologists Dr. Gulshan Mukhiya & Dr. Sandeep Morkhandiker.
The patient, Prabhu Lal Purohit (Age 55 years) was suffering from pain in abdomen and constipation. On his evaluation there was a large pulsation mass in the abdomen with severe anemia. USG Abdomen was done which states that aneurysm has started bleeding from inside. Due to which the patient could not pass urine and was encountered with further problems in kidney. The patient was already informed about aneurysm two years back which was treated by medicines. Since then the disease has become severe and the patient was admitted in emergency condition. On further discussion with Nephrologists CT scan was done along with dialysis of the patient and he was operated on emergency basis. Since the patient’s condition was very risky so endo vascular procedure was not possible and he was operated by open surgery which lasted 7 hours. In the surgery the abdomen was opened and aneurysm was directed to bypass machine and the aorta was repaired from upper abdomen to both side of the legs and the abdomen was closed. The patient was then shifted to ICU where post operatively the patient recovered very well. He didn’t require dialysis thereafter said Dr. Gandhi.
In a population of 1 lac people only 4-5 patients suffer from the mentioned disease. The success rate of Endo Vascular Procedure is 90-95% and the Open Surgery for ruptured aneurysm is 505. The treatment facility in Udaipur stands second after Jaipur in Rajasthan. Also the patient should not suffer from any of the critical disease as cancer, congenital heart disease, liver, kidney and pancreas proper functioning etc. Also the hospital should be equipped with cardiac operation theatre, Cardiologist, Cardiac Surgeons, Cardiac Anesthetists, Neuro & Vascular Interventional Radiologist, Nephrologists, Cath Lab, ICU and all types of surgery facilities.
What is Endo Vascular & Open Surgery?
Endovascular surgery is an innovative, less invasive procedure used to treat problems affecting the blood vessels, such as an aneurysm, which is a swelling or “ballooning” of the blood vessel. The surgery involves making a small incision near each hip to access the blood vessels. Open-heart surgery is any type of surgery where the chest is cut open and surgery is performed on the muscles, valves, or arteries of the heart.
